Age: 17-80; measures feelings about the future, loss of motivation, & expectation; assesses the client’s negative attitude about the future; can be used to evaluate suicide risk

Carroll Depression Scale (CDS-R, CRS)

Age: 18+; self-rating instrument for the presence & severity of depression

Zung Self-Rating Depression Scale (SDS)

Age: N/A; used to assess depression levels in clients diagnosed with a depressive disorder

Hamilton Rating Scale for Depression (HAM-D or HRSD)

Age: 18+; rates the severity of symptoms observed in depression such as low mood, insomnia, agitation, anxiety and weight loss; used to evaluate the effectiveness of depression treatments

IPAT Depression Scale

Age: 16+; used to diagnose depression; based on 7 depression factors including somatic complaints, feeling of guilt & excessive self-criticism

Child Depression Inventory 2 (CDI 2)

Age: 7-17; assesses cognitive, affective, and behavioral symptoms of depression in children and adolescents

Short Mood and Feelings Questionnaire (SMFQ)

Age: 6+; screening tool for depression in children; also helpful in measuring symptom severity & treatment progress over time
